Please find attached the following forms that can be printed and brought in to Gateway Co-op.

Application for Employment — To be used for any applications for employment with Gateway Co-op.

Request for Donation — To be used by organizations that are requesting donations of any form.

Equity Application & Name Change Application — To be used for all changes and applications regarding Gateway Co-op membership including;

Name changes
Adding or removing an individual to the membership\
Withdrawal requests including;
- For deceased members
- Members over the age of 70
- Members who are moving out of our trading area
